Our Aging Life Care Professionals® work with older adults and their families as personal care managers, developing plans for support and overseeing its delivery.
Your Care Manager will:
- Evaluate and develop a customized care plan to perfectly address your needs
- Coordinate care that encourages independence, yet address any safety issues
- Accompany clients to physician appointments and provide families with details and important information
- Assist with communication between clients, their families and their healthcare providers
- Oversee the care plan and act as a local safety net for loved ones living far away

Medical Care by Cypress at Home provides the care that’s just right for you through in-office, in-home and virtual-based visits. Medical Director Dr. Bob Rahmanian and his team of Physician Assistants and Nurse Practitioners are committed to being the care team you can trust to help you age successfully at home.
Our medical team available for routine healthcare needs, i.e. check-ups, annual physicals, health screenings and on-going care for conditions such as diabetes, hypertension or asthma and non-urgent needs such as colds and flu. We accept major insurances and private pay.
Our Home Health services will help you or your loved one remain independent and free from unnecessary hospitalization. A certified Medical Director supervises our medical in-home care and our highly trained on-call nurses are available 24 hours a day, 7 days a week.
Our state of Florida licensed nurses and certified nursing assistants are highly trained in both care and customer service. This training means the perfect personalized care plan is delivered right to you in the comfort of your own home.
